Confidentiality

All private health information (PHI) is confidential and is not released without the written consent of the patient. PHI or patient identifying information will not be divulged to any source, unless otherwise required by law, without a signed release of information initiated by the patient.  Psychiatry 280 is compliant with the Health Insurance Portability and Accountability Act (HIPAA).

Cancellation Policy

Patients need to cancel their appointment 24 hours in advance or they will be charged a $50 no show fee. If they can’t reach us, please leave a voicemail the day before.

Late Arrivals

If a patient is 15 min. late to their appointment they will be marked as a no show. Please call if you are going to be late and we will see what we can do.

No shows

No show fees have to be paid in full before being rescheduled.

Prescriptions

Patients are expected to keep prescriptions of any controlled medications in a safe place.  Lost or stolen prescriptions will be refilled at the discretion of the physician and may necessitate the filing of a police report.

Emergencies

Calls to the emergency line are for Emergencies only. If it’s not a true emergency, call the office and leave a message with the front office staff and the doctor will respond to your message within 1 business day.

Financial

All balances and co-pays have to be paid in full before being seen by the doctor. If you cannot pay in full, we will have to reschedule the appointment.

Patient Age Policy

The majority of our patients are adults ages 19 and older. Some exceptions can be made upon prior approval from the doctor.

New Patient Forms

All new patients must fill out all the forms and send them to us along with a copy of their driver’s license and front and back of their insurance cards before they can be put on the schedule

Walk-ins

We do not accept walk-ins.

Forms of payment

We do not take checks at the time of apt. We do accept cash or debit/credit cards. We only accept checks when they are mailed in for balances after receiving a statement.

Insurance referrals

If your insurance requires a referral, it is your responsibility to contact your primary care provider and have a referral faxed to our office prior to your apt.

Self-pay

New patients that do not have insurance must be pre-approved by the doctor before being added to the schedule.
